When AI Signs Prescriptions

Utah has approved a pilot that allows artificial intelligence to renew certain prescriptions without a doctor actively signing each order. The system, built by Doctronic, handles routine refills for patients already diagnosed and prescribed medication by a physician. It asks follow up questions, checks records, screens for interactions, and either renews the medication or routes the case to a human clinician. State officials say the goal is speed and access, especially for patients who wait days or weeks for basic renewals. As one regulator put it, “This is about removing friction, not removing care.”

The scope is intentionally narrow. Roughly 190 common medications are included, such as blood pressure drugs, antidepressants, and birth control. Controlled substances and new diagnoses are excluded. According to the company, internal testing showed the AI matched physician decisions more than 99 percent of the time. Utah framed the program as a supervised experiment inside its regulatory sandbox, a space designed to test emerging technology without rewriting the entire healthcare system at once.

Medical groups remain cautious. Leaders at the American Medical Association warn that automation can miss context that surfaces only through human interaction. Federal oversight is also unresolved, as the Food and Drug Administration has not set explicit rules for autonomous prescribing systems. Still, the experiment reflects a broader shift: AI is moving from advisory roles into decision making, forcing society to renegotiate trust, responsibility, and the pace of modern life.
